Claims on Behalf of Another Person/Joint/Multiple Owners

Steps on Claiming on Behalf of Another Person

  1. You can submit claims as a parent, custodian, conservator, trustee, guardian, power of attorney, or legal representative on behalf of another person. 
  2. Enter the name of the person you are claiming for in the online search Locating Unclaimed Property Owners
  3. Once you locate the name for which you wish to claim, click on the claim form COT ST-912.
  4. Complete the information in Part A and print it out. Once printed please sign in Part D.
  5. Attach proof of address or ownership connecting the owner to the funds.
  6. Provide the following documents as they apply:
    • Court documentation showing the authority to act on the original owner's behalf.
    • Power of Attorney documentation showing the original owner is living and you are the designated Power of Attorney.
    • Copy of birth certificate of minor child.
    • Copy of your photo ID. 
    • Copy of Social Security Card for minor & custodian. 

Steps on Claiming Joint Accounts or Multiple Owner Accounts

  1. You can submit claims for joint accounts or multiple owner accounts. 
  2. Enter the name(s) of the person you are claiming for in the online search Locating Unclaimed Property Owners.
  3. Once you locate the name(s) for which you wish to claim, click on the claim form COT ST-912.
  4. Complete the information in Part A and print it out. Once printed please sign in Part D.
  5. If it is a Joint Account, both owners must sign the claim form, as well as submit copies of photo ID, proof of SSN and proof of ownership or address listed. 
  6. If one owner is deceased, or in the case of divorce or any other change associated with the account, please submit supporting documentation.
